Doctors have obligations to fulfill, and one of them is to save lives regardless of the patient’s background. Monster is a slowburn psychological thriller anime that follows the life of Dr. Kenzo Tenma, a talented neurosurgeon whose decision to save a young boy, Johan Liebert, instead of a prominent political figure, sets off a chain of terrifying events. The prominent figure unfortunately dies while Johan survives and grows up to become a manipulative and psychopathic serial killer, and Tenma, burdened by guilt, embarks on a quest to stop him. There is no way Tenma would have known that his decision would have such terrifying consequences, but ethically and morally, he did the right thing by saving Johan’s life and not giving preferential treatment to a prominent figure as instructed by the hospital director.

Monster unfolds gradually, focusing on deep character development, especially Tenma's internal conflict over the morality of his actions. As Tenma pursues Johan across Europe, Johan’s dark past that shaped him is slowly revealed, exploring themes of good versus evil, the value of human life, and the psychological impact of trauma. This anime builds tension through quiet moments and psychological confrontations rather than fast-paced action, making it a classic example of a slowburn story where suspense and moral dilemmas gradually intensify before reaching a powerful climax.
