First, you must get Sins of the Flesh content by performing a sermon. It will unlock rituals and new buildings for you, including the Mating Tent. In addition, you will also need a Drinkhouse or Drum Circle to cultivate sins in your followers.
Once you have built the Mating Tent, interact with it to select the two followers with the highest chance of mating. You can then choose which traits parents will pass on to their child.
