After completing the Stained Honor quest in Kuttenburg City, talk to Voyta about his past. He will tell you to visit a dugout located northeast of Maleshov. Once the conversation is over, fast travel to either Maleshov or Danemark points and ride towards the marker. As you reach the zone, you can find a man sitting next to the dugout.
When you approach him, he will become hostile, and to defuse the situation, you can surrender to have a proper conversation with him. He will talk about Voyta being dead, then hand you the Madman's broken sword. Your objective will change, and you will have to investigate the dugout. In order to acquire the trophy/achievement tied to this quest, you must leave this place before investigating and make your way to either Miskowitz or Kuttenburg, as they are the closest, to reforge the weapon at a blacksmith.

To reforge the Madman's Longsword, you need 3 Frankfurt Steel and 1 Fastening Material, all of which should be available to purchase from the blacksmith near the anvil. Start hammering your workpiece, making sure you do not hammer the same spot multiple times.
If you are overheating the workpiece, Henry will comment on it, so make sure you are listening to whatever he points out.
Investigating the Dugout In Kingdom Come: Deliverance 2
Once you are done reforging it, take it back to the man and tell him that you fixed his broken sword. At first, he will refuse to take it, and you will have to pass a Skill Check if you want him to keep it. With the sword back with its original owner, you can start your investigation. Look around the scarecrow in an anthill, then look towards the noose on a branch. Head inside the dugout and investigate the wolf's heart on the plate. When the objective changes, you must head back to Voyta in Kuttenburg.
After you are done speaking with Voyta and complete the quest, wait 2–3 days. After that, make your way back to the dugout for the last time. Upon arriving, you will find out that the man has taken his life by hanging himself from the noose. This should pop the Curiosity Killed the Cat trophy/achievement, and you can grab the Madman's Sword from the chest inside the dugout.
