An annoying problem plaguing Fallout 4 is fixed ahead of the post-apocalyptic RPG's upcoming April 25 update.
The following is a guide on how to create a basic Infiltrator build that uses VATS to deal massive critical damage with pistols as the weapon of choice. While it is somewhat of an underrated build in Fallout 4, the Infiltrator setup is great for those who really want to dig into the secret agent/assassin fantasy.

Agility dictates how much AP you get as well as your overall effectiveness when sneaking. Meanwhile, Perception increases your VATS accuracy, which is something players will want later on. These two can be considered the best SPECIAL stats in Fallout 4 when it comes to making gun-based builds.

The list above shows the main perks you should get when planning a Fallout 4 Infiltrator build. This setup will greatly increase the damage you deal, especially when sneaking and using critical attacks via VATS. Since Gunslinger and Bloody Mess don't need any special conditions to activate, they'll be useful even outside of stealth.
Locksmith and Hacker aren't exactly mandatory, though they will open up tons of areas for you when exploring the wasteland, given that you know how to pick locks or hack terminals in Fallout 4.
Prioritize getting these perks above everything else. If you have no other option, invest your points in SPECIAL stats or other quality-of-life perks such as Scrounger or Gun Nut. If you want to use rifles, the Rifleman perk under the Perception stat will be open to you as early as level 1.

Fallout 4 Infiltrator - Recommended Gear
Early on, any silenced weapon will do. You don't strictly need to use pistols, but once you do make enough progress in the game and get the Gunslinger perk maxed out, you'll need to use a good hand gun in order to scale well.
The Deliverer is one of the best weapons in Fallout 4. You can get it after completing Tradecraft, which is the second quest in the greater Railroad quest line. Apart from coming with a built-in suppressor, this gun also has a higher chance to hit when using VATS, and it uses 25% less AP than normal.
For armor, you can wear whatever you want. However, if you want to further improve your ability to sneak, use a full set with the Shadowed mod for all pieces. This will make you even harder to spot.
While you're exploring, keep an eye out for US Covert Operations Manual magazines and the Sneak Bobblehead in Dunwich Borers to become even harder to detect.
