Tribal Medicine is a side quest in Path of Exile 2 that asks players to “Find a Use for the Shark Fin,” after they obtain the item at one of the islands in Act 4. The game offers no hints as to the exact purpose of the item, who needs it, and what to do with it, leaving players floundering in search of answers.
What’s even more troublesome is that the Shark Fin takes up a lot of inventory space, which makes carrying loot back and forth between maps more tedious than it needs to be. Luckily, it is very easy to complete Tribal Medicine if you know where to look. Here’s the breakdown.

Tribal Medicine Quest Guide
The Shark Fin can be delivered to an NPC named Kaimana in Kingsmarch in Act 4 of Path of Exile 2. Kaimana is a tribal healer stationed on the Karui ship docked directly opposite Makoru’s ship.
Cross the pier and you’ll see the Karui ship on the southern side, with Kaimana standing near its main mast, behind Tavakai (pictured above).
Upon giving Kaimana the Shark Fin, she will offer players three possible rewards.
- Level 11 Uncut Skill Gem
- Level 11 Uncut Spirit Gem
- Level 4 Uncut Support Gem
Choose one of the rewards to complete the Tribal Medicine quest.
Kaimana Missing From Kingsmarch
The Karui ship and Kaimana are not always stationed in Kingsmarch, making it impossible to complete Tribal Medicine. If this happens, it means Kaimana has moved to Ngakanu Island temporarily. Progress through the main story of PoE 2 in Act 4 to unlock and clear Ngakanu Island to have Kaimana return to Kingsmarch, allowing players to turn in the Shark Fin to complete the quest.

The Shark Fin is a guaranteed drop for defeating the “Great White One” shark boss on Whakapanu Island in Path of Exile 2. The great white shark can be found at the edges of the map, in a large sandy arena called the Shark Pit near the ocean, where it emerges when players approach.
