Every charm in Mafia: The Old Country can serve a purpose, but some are objectively a lot better than others. Given how many stealth sections there are in the game, players should definitely consider equipping the Fantasma bead after unlocking it in Chapter 4. For ranged combat, Paziente and Acuto pair well together and can make aiming much easier. For their last bead, players should either go with Destrezza to increase their reload speed or Brutale for a damage boost across all ranged weapons. San Bernardo da Corleone is arguably the best medallion, particularly when playing on harder difficulty settings, although those who plan on looting every lockbox may prefer the San Francesco d'Assisi, especially if their blade of choice has low durability by default.
