Easy

Medium

A balanced experience for players who want a fair fight. Your weapons deal standard damage, enemies have normal accuracy, and vehicles take moderate damage from collisions.

Hard

A tougher experience for players looking to test their skills. Your weapons deal less damage, enemies hit harder and with higher accuracy, you gain fewer resources from looting and pickups, and vehicles take realistic damage from collisions.

Players will be able to change the difficulty at any time via the Gameplay menu. There is a Trophy / Achievement for completing the game on Hard mode, although this can be unlocked by completing the final chapter again via the Chapter Replay menu.

For the average player, Medium is the best Mafia: The Old Country difficulty setting. Hard arguably offers a more realistic experience when it comes to the damage sustained by the player, but it massively nerfs the damage that they inflict, which can make enemies feel a bit too spongy at times. The handling of some of the cars is also a little floaty, so taking more damage from collisions is the last thing most players will want. In an ideal world, the devs would allow players to adjust some of these parameters separately so that both enemies and Enzo go down after being shot two or three times, but this isn't possible at launch.
