Like the other Unique items in the game, Sacrosanctum can drop randomly from pretty much everywhere. Unfortunately, this item cannot be target farmed, though do keep in mind that you need to be at least level 68 to use it. This also means that you need to be close to or have exceeded that level before it can drop for you.

The Vaal Temple introduced in the Fate of the Vaal league can potentially give players a Sacrosanctum drop. Not only can players improve the drop rates for rare items within the temple, but they may also place rooms that contain Unique items. If you're lucky, you may end up getting a Vaal Temple room that contains a guaranteed Sacrosanctum drop, or you may get one that drops a random Unique.

The easiest way to get a Sacrosanctum in PoE 2 is to just buy one from other players. Open up the in-game Market Panel, search for Sacrosanctum, and you should be able to find a list of players selling their copies of the item. Just keep in mind that prices tend to fluctuate based on player demand. If you're lucky, you may find a Sacrosanctum for just 1 Exalted Orb. If not, it could cost more.

Sacrosanctum's main draw is its ability to recoup both Life and Energy Shield. For the unaware, the recoup effect replenishes your Life by a certain amount after you take damage. With Sacrosanctum, you'll also recoup your Energy Shield, which can lead to some very interesting interactions.

One of the most popular uses of Sacrosanctum is with the Infernalist's Pyromantic Pact passive. The latter replaces your Mana with Infernal Flame, which builds up to a maximum cap. When you reach max Infernal Flame, you take Max Life and Energy Shield damage, and your Infernal Flame count is reset to 0. With Sacrosanctum, you'll recoup both Life and Energy Shield whenever Pyromantic Pact damages you.

This works with any effect that damages you (like Self-Immolation) and passives that affect Recoup (like Chronomancer's Rapid River).
