I think there are just server issues sometimes...I actually think having a very fast connection can sometimes be detrimental as well as there may be over compensation for less speedy connected players.
I have logged over 100 hours and know the time to kill on all guns but there are times when someone's turns a corner and I'm instaneously dead...like there's no sound of shots hitting me at all but in the kill cam, you can see I'm getting hit multiple times. My ping is under 25 most games so when this happens and I'm getting shot with a c9 or scar, I know its a server or connection thing and I'll just leave the game before sacrificing my sanity.
