W Item technically doesn't need 2 of the item, just one. You can select a different item the second time.

Hel, technically you don't even need 1 of the first item if you can add it to your inventory mid battle via a command such as Morph.

Also technically some enemies in Final Fantasy VI were immune to Vanish.
