Twisted Nerve sends the LSPD SWAT unit to 213 Park Homes to raid the location of a possible meth operation. This is one of the earlier levels of Ready or Not, and players can expect a lot of room-clearing in very tight spaces against drugged-up junkies, meth cooks, and their hired muscle.

This mission isn't terribly hard to finish, but its objectives can be easy to miss, especially considering all the clutter in the two houses within the area of operations. Here's a guide that covers the locations of all objectives as well as some tips on how to get through the mission more smoothly.

Updated August 5, 2025 by Marc Santos: The console launch of Ready or Not also brought about a number of changes to the base game, including the addition of difficulty levels and some tweaks to how NPCs work. The flow of the game should still feel the same as before, but to round things off, we've updated this guide to include a section on how to get S-rank on Twisted Nerve while keeping the console launch update's adjustments in mind.

Ready or Not Player Limit: How to Play with More People

Looking to get more people into your Ready or Not lobbies? Here's to squeeze more players into your SWAT team.

First House Objectives

Important Objectives

  • Incapacitated minor in the child's bedroom
  • Meth lab
  • Money cache

Starting with the first house on the left, players have three objectives to find: the incapacitated minor, the money cache, and the meth storage. Luckily, all of these are right next to each other, and players should have no trouble finding them.

Breach the main door of the house and clear the first floor. There are usually suspects inside the guest room and the living room. Use an optiwand to check each of the rooms and deal with the situation appropriately. Then, head upstairs to continue with the mission.

All three of the objectives are in the rooms by the hallway behind the staircase. The left one leads to the meth storage, the one on the right is the money cache, and the one at the end of the hall leads to a series of rooms leading to the incapacitated child. Clear each room and tag all the objectives. After this, you may want to clear the backrooms downstairs first before crossing the bridge to the next house. Neutralize all suspects, then return to the second floor.

Second House: Crystal Meth Labs

From the second floor of the first house, players will be able to see the first crystal meth lab right off the bat. Breach and clear, and make sure to check the attic too. Once both rooms are cleared, go out into the hallway.

The second crystal meth lab is directly in front of the first one. Open the first door on the left, then interact with the meth cooking equipment on the small table by the window. After this, carefully clear out the rest of the map to complete the mission. Luckily, players no longer need to look for meth packages in Twisted Nerve like in the early access version of the game.

A few tips to clear this mission more efficiently:

  • Check under the beds for hidden suspects and civilians.
  • Do not throw flashbangs in the incapacitated minor's room.
  • Use shotguns and aim at the upper chest for faster takedowns.
  • Beware of civilians with knives
  • Wear heavy ceramic armor to survive gunfights in long corridors

How to Get S-rank in Ready or Not: Twisted Nerve Mission

Ready or Not Steel Armor description

While Twisted Nerve is a fairly easy mission to complete, getting that perfect S-rank on it can be a little tricky. The tight spaces inside the houses and the tunnels make for very tense encounters, and since players can't afford to make mistakes during an S-rank run, things can get quite hectic rather quickly. Below are tips and strategies for you to try:

  • Bring non-lethal weapons. Killing suspects will disqualify you from getting an S rating. Pepperball launchers are strongly recommended, as beanbag shotguns can potentially kill a target if you hit their head up close.
  • Clear the outer perimeter first with NVGs. The house and garage in the backyard can be tricky to clear due to how dark the area is, so bring nightvision goggles to make things easier.
  • Shoot first, yell later. If you're using a non-lethal firearm, you can shoot armed suspects even without yelling at them for compliance first. Still, try not to pepperball someone who isn't carrying a weapon.
  • Save grenades for the crystal meth lab and tunnels. The main house is easy enough to clear with just weapons, but these two are not.
  • Ditch the bots. AI squadmates are much more effective following RoN's console launch, but if you're having trouble clearing any mission on S-rank because one of your AI squadmates keep getting shot, consider playing solo or with other players.
