From the Chain Box, head south and enter the Restroom. There should be a cart that you can climb and head into Room L1. A breakable wall in Room M3 lets you access Room M2, where you will find the Calendar Page. The Calendar Page gives the first number, which is "1".
Follow these steps for the remaining three numbers:
- Look inside the Sink to find six pills
- Turn around to find a hole in the wall that shows a light flicker twice before going dark
- Through the door slit, you can see a clock that you can approach from the other side, and upon turning the hand to "2," the clock will start ringing.
Matching the numbers with the Calendar Page, the code for the Combination Lock you get is 1622.
Where to Find The Lapis Eye Key
From the Chained Box in front of you, head left to encounter multiple enemies and a staircase that takes you up to 3F. Head into the X-Ray room to find a mannequin sitting on a surgical chair. Follow these steps to find the Lapis Eye Key:
- Pick up the Instrument of Force and interact with the Mannequin to break its right arm.
- Deal with the Bubble Head Nurses and go north of Room D2 to break the wall with your Metal Pipe.
- Head down the ladder in Room D1 and go into Medical Records.
- Find a gap you can squeeze through to reach the Restroom and pick up the Key of Bliss.
- Use the Key of Bliss on the Mannequin to acquire the Lapis Eye Key.
How to Get The Keypad Code
This time from the Chained Box, head right to the Director's Office. There should be a breakable wall that leads into the Laundry Room. Push the metal cabinet inside Room L2 and move the metal cart on your left to head inside Room L3. You can find the Director's Office from the Observation Room next to the TV.
Return to the Director's Office and head up the ladder in the middle of the room. To your left is a Treatment Strecher with no power, so follow the red wire to start the Generator. With the Generator up and running, head back to the Treatment Stretcher and press the button to reveal the number "9659," which is the code for the Keypad.
