Smile 2 is both a direct and an indirect sequel to the original film. Besides the intro tying into Joel from the first film, the main plot revolves around a pop star making a comeback. Skye had some problems with substance abuse which led to a traumatic event.
She’s on a new tour to showcase she still has the juice to be an idol but something keeps getting in her way. Is it her own self-doubts or is some curse behind it all? Either way, her journey leads to some truly grotesque deaths in Smile 2 that rival that of Alien: Romulus.

6 The Mobsters
Pumpkin Spice And Knife Splice
The movie opens with Joel from the first movie sitting in his car, waiting to ambush a man with Pumpkin Spice Frappuccinos. He leads the man inside a house to see what Joel assumes is the head mobster. After a quick knife kill, which is quite visceral, a gun battle ensues with a lot more mobsters.
It’s not the exact way Joel wanted this to go down as he presumably planned to lift the curse. Instead, he has to hide from other mobsters that arrive after the men he assaulted die. This intro demonstrates that it is a much gorier movie than the original or other media and one that will overall not pull its punches.

Got Pancaked By A Truck
While Paul did not die the way he wanted in that mob house, he did indeed die shortly after. In his escape, he runs into the middle of the road wherein a truck absolutely destroys him. Other horror movies may be content showing the blood trail and leaving the death up to imagination but that’s not the case here.

Viewers will learn early on that Skye, the protagonist, was in a car accident. This killed her actor boyfriend, Paul, and left her with intense scars on her back and chest. She also has crippling pain that won’t go away and normal pain pills can’t solve her issues.
She can’t get anything harder except through drug dealers because of her sketchy past. When she runs out of the good stuff, Skye calls her old dealer Lewis. When she goes over there, Lewis is hysterical which eventually leads to an episode wherein he sees some kind of horrific monster. Then he kills himself with a detached weight and they show everything.
3 Morris’ Brother
He Must Have Played A Lot Of Half-Life
There’s one super quick kill in the movie that they don’t spend much time on. Throughout the experience, Skye keeps getting texts from an unknown number. Because this murderous parasite thing makes people see illusions, the audience is led to believe these texts are all in Skye’s head.
They are real though which leads Skye to a man named Morris. He’s a nurse who wants to help Skye end this curse because it took his brother. As he tells her, the movie shows a quick cut to Morris’ brother removing his own jaw with a crowbar. What would make him use a crowbar unless he played a lot of Half-Life?
2 Elizabeth
Using Mirror Shards Destructively
Skye’s mom, Elizabeth, seems to only care about one thing in Smile 2: Skye going on tour. Even though she is showing erratic behavior, she chooses not to see it. This all comes to a climax after Skye wakes up after an accident.

She’s in a hospital bed, next to her mom, and says she can’t do the music tour anymore. After an argument, her mom changes into one of those parasitic humans. She breaks the mirror and starts cutting up, going as far as to remove her eye with a shard. It’s unpleasant, to say the least, but Skye discovers she was the one who killed her or so it seems.

The end of the movie leaves fans with one big mystery. How much of the movie post-Lewis’ death was an illusion? The final scene of the movie shows Skye on stage with her mom cheering from beyond with the rest of her supporters. Is that an illusion or was everything else an illusion?
Horror movies that end like this can be frustrating but it is also fun to discuss with friends and family after. Illusion or not, Skye goes through the wringer onstage as she faces some Elden Ring-level demon entering her horrifically stretched-out mouth and then she stabs herself with a microphone onstage. Her fans do not look pleased with their concert of choice.
