Players will have to choose to help Abhartach or not, and attend to his particular request of letting players 'bite them.' They will meet this dark and sinister character while completing the 'I See Darkness' Quest, and its proposal still puzzles even the most hardcore adventurers. How To Start I See Darkness In Tainted Grail: The Fall of Avalon
When players are visiting the Village in Tainted Grail: The Fall of Avalon, they will spot a man in a cage, and a woman besides him. By speaking to Betrys, they will learn about a strange disease that has been spreading from one of the refugees into the Keepers. If they promise to investigate the source, the quest I See Darkness will start, and players will have to find the origin of the strange disease and figure out if they can save the Keeper and the afflicted villager.
Betrys will direct the players to a camping spot near the King's Road, and players can find the exact place by following the quest marker and heading north of the fortress. When they reach the camp, they will spot a Corpse Eater lurking around, dispose of it, and approach the sleeping mat behind the quest marker, then sleep to enter the dream world. And that's when players will come into contact with the mysterious being known as Abhartach.

Following the mysterious voice down this dream world's road, they will find one of the Fore-Dwellers named Abhartach, chained in the bottom of a dark pit. He will beckon the player to come closer and explain that he needs only 'one bite'. If questioned, he will assure the players that he can restore the afflicted people to normal. Now, here comes the important choice.
Should Players Let Abhartach Bite Them Or Not?
If players allow Abhartach to take some of their blood by biting them, they will take a little bit of damage, and Abhartach will be immediately poisoned by Arthur's blood (running through the veins of the main character) and die.
If the players refuse, Abhartach will become enraged and try to kill the players. A Boss fight will begin, but the weakened Fore-Dweller cannot do much except attack once and immediately fall to his knees due to exhaustion.
The first option finishes the interaction without the need for a boss fight, and the players will be able to loot Abhartach's body and keep the Fore-Dweller's Bauble (a magic focus) for themselves. Same if they choose to reject Abartach and kill him, the experience and the loot will be the same.
How to Get Rose, Sage, and Feather in Tainted Grail: The Fall of Avalon

No matter what players choose in this case, the consequences will only vary during their encounter with Abhartach. They can skip the boss fight or choose to face him in combat. Once they return to the Village to check on the patients, Betrys will tell them about Mahon (the affected Keeper) having recovered from the disease. Col, her beloved, isn't as lucky and remains afflicted and catatonic.
There's yet another choice here that players need to make. If they have a higher Spirituality attribute, they can choose to convince Betrys to let them 'end Col's suffering,' which will yield some extra gold and exp as a reward, and finish the I See Darkness Quest.
Ending Col's life doesn't have major implications for the plot, however, if players return after a few days to the place where Betrys and Col live, they will find the house vacant, and the mystery of her disappearance still unsolved. It is possible that Betrys took her own life, following her husband to the afterlife.
So, the choice here seems pretty much a bad ending for all decisions: leaving Col alive would potentially cause Betrys to have to take care of her crippled husband for life, taking care of him in her stead ends with both NPCs disappearing, while forcing Betrys to take care of ending Col's life would displease Arthur, and cause players to have a significant reputation drop.
If players fail to persuade Betrys, the quest will end there, and they will be locked out of the additional objective. If players manage to persuade Betrys, she will ask them to kill Col during the night. Do so to prevent raising a bounty and to complete the quest successfully.
